Forms Development

Product Overview:
	With the use of HCL Domino/Notes software (previously an IBM Product) and BTI's programming staff, your organization can convert paper forms into electronic forms. Integrated with BTI's HSS product, this process will be a time saving device for your organization. Utilizing a Domino Server for the IBM Power Server and Notes Client on each PC which will use the application, your agency will cut down on paper forms and increase and improve the quality of data capture by using these electronic forms and Notes. Several of our customers have implemented an entire medical record to eliminate used of their paper based chart.

	Software Needed:
		HCL Domino Server - (for BTI/HSS integration at least one server should reside on the IBM Power Server hosting HSS)
		HCL Notes Client on each PC which will use the application.

	Project Specifications:
		Convert supplied paper forms to the equivalent HCL Notes electronic entry forms.
		Provide graphical user controls to accelerate and improve quality of data capture.
		Maintain a central repository and archive on i5/Power Domino server.
		Provide print output options to be reasonable facsimile of original paper form.
		Priority population assessment forms will include scoring logic and data entry validation logic.
		Provide mechanism for selected demographic data transfer to HSS Client Database on i5/Power Server.
		Archive/Deletion logic of processed forms.
		Form Review and Approval.
		Clinicians can create and maintain their own form database on detached PC’s (laptops, etc.). No need to remain connected to network.
		Data is synchronized on demand by Domino's e-mail infrastructure. Automated Export / Import of Client Classification Data to HSS database.
		Form data from the Client Information Form (and other related forms) will populate respective data elements in the HSS client database.
		Security issues: mechanisms will be in place for data security and data encryption.
		Import, export, and transfer mechanisms will be in place for assigning cases to alternate clinicians.
		Easy copy, paste, edit procedures.
		Crosswalk of data element codes provided as required by HSS description master table.
		Automated agents and iSeries Service function to process new forms.
